When doctors order these tests
They fall into two groups.
Risk tests, for people who feel well but have diabetes, high BP, a family history or a smoking habit. The lipid profile test comes first. If the picture is unclear, doctors may add a lipoprotein a test (a genetic marker, done once), an apolipoprotein test for particle numbers, an hs crp test for artery inflammation, or a homocysteine test, which is often linked with low B12. Many of these come together in our Cardiac Risk Profile.
Injury and strain tests, ordered after symptoms. The troponin test is the main marker of heart muscle damage, and the ck mb test adds timing. A cpk test mostly reflects skeletal muscle and is checked on statins, and the LDH test is a general tissue-damage marker. The NT-proBNP test looks for heart strain in breathlessness or swelling.
Preparation
Fasting rules differ. Lipid profile, apolipoproteins, homocysteine and the cardiac profile need a 10 to 12 hour overnight fast, with water only; tea breaks the fast. Lp(a), hs-CRP, troponin, CK-MB, CPK, LDH and NT-proBNP don't need fasting.
Avoid hard exercise for two days before CPK, CK-MB or LDH. Wait two weeks after a cold or fever for hs-CRP. Mention biotin supplements for troponin and NT-proBNP. Routine tests are usually reported the same day if given by about 11 am; specialised ones are collected here and processed in-house or at a partner reference laboratory, usually in 1–3 working days.

FAQ
Which cholesterol test should I ask for?
The lipid profile, unless your doctor writes something else. It gives total cholesterol, LDL, HDL and triglycerides from one fasting sample.
Can a blood test tell me if I'm having a heart attack?
Troponin helps doctors decide, but only with an ECG and repeat readings in a hospital. With symptoms now, go to emergency.
Do you do an ECG?
No. Alpha Laboratory is a pathology lab, so we do blood tests only.
Can I get these tests at home?
Most of them, yes, in Banur and nearby villages. Homocysteine is better given at the lab because the sample must be separated quickly.
